60-year old patient, high-speed road accident, no other clinical data.

 

Learning to read a body scan.

Findings

Cranioencephalic space:Subarachnoid hemorrhage in the parafalcine cortical sulci of the vertex, right temporal and occipital sulci, and in the interpeduncular cistern.Normal-sized ventricles.No deviations of median line structures.No other intracerebral posttraumatic lesions.

 

Spinal space:No posttraumatic lesions.

 

Thoracic space:Small volume of right pneumothorax.Moderate anterior hemomediastinum associated with non-displaced transverse fracture of the distal third of the body of the sternum.No large vessel lesions.Bilateral posterobasal ventilation disturbances.

 

Abdominopelvic space:No pneumoperitoneum.No solid organ lesions (benign cortical cysts on the left kidney).No intraperitoneal effusion.No gastrointestinal parietal thickening.

Subarachnoid Hemorrhage, Traumatic, Pneumothorax, Sternum fracture

Discussion

FOLLOW-UP:

 

Patient treated with standard monitoring; favorable progression.

 


Key points

Important points:

 

- Nine times out of ten, posttraumatic hemomediastinum is not caused by an aortic lesion

- Triangular hyperdensity at the interpeduncular cistern indicates the presence of SAH.
