小脑半球第V小叶(又称第V[H]小叶)是小脑半球前叶的组成部分。
根据Larsell的划分,前叶包含小脑半球的五个小叶(第I至第V[H]小叶)。这些小叶的形成源于多条横行裂隙的出现,这些裂隙沿内外侧平面延伸贯穿整个小脑半球与蚓部。小脑半球第V小叶与第IV[H]小叶共同被称为"前方形小叶"(依据经典命名法)。
从功能角度而言,小脑半球第I至第V[H]小叶连同蚓部(即前叶)是脊髓小脑传入通路的主要接收区域。这些传导束将感觉本体感觉信息传入小脑,使其能够实时感知各身体部位的运动状态与空间位置。小脑将这一实时本体感觉信息与最初由大脑制定的运动计划进行匹配与比较,从而在潜意识水平上对随意运动活动的监控与精细调节发挥关键作用。
