前丘脑网状神经核由一薄层灰质构成，夹于内侧的丘脑与外侧的内囊后肢之间。这是唯一不向大脑皮质投射的丘脑神经核。传入输入来自大脑皮质，为谷氨酸能／兴奋性纤维。而传出纤维则由γ-氨基丁酸能／抑制性神经元组成，投射至其他丘脑神经核，从而调节丘脑活动。前丘脑网状神经核还接受来自脑干网状结构及底部前脑的额外输入，可能参与调控觉醒状态与注意功能。
