椎动脉的椎前部；V1段（孔前段）自锁骨下动脉发出处延伸至C6横突孔。

该段向上向后走行于颈长肌与前斜角肌之间。其前方有颈内静脉和椎静脉，并有甲状腺下动脉横越其前；左侧椎动脉另有胸导管横越其前。其后方有第七颈椎横突、交感干及其颈下神经节。
