中间中央质由围绕脊髓中央管的灰质构成，对应Rexed第十层（板层X）。它分为背侧连合和腹侧连合，分别位于中央管的后方和前方。这些连合将脊髓左右两半的灰质相互连接。

在外侧，中间中央质延续为外侧中间质，也称为后角底部或第七层（板层VII）。该区域位于脊髓灰质后角与前角之间。中间中央质与外侧中间质共同构成脊髓中间带。
