中央后沟是位于顶叶的一条重要脑沟。它与中央沟平行走行，位于其后方，自大脑半球上缘向下向前延伸。它与中央沟共同构成中央后回的边界，中央后回亦称初级感觉区。中央后沟还延伸至大脑半球内侧面，在此与扣带沟的缘支相连，形成中央旁小叶的后缘。
