Le sillon médian postérieur (sulcus medianus posterior) est un sillon peu profond qui s'étend le long de la ligne médiane de la surface postérieure (dorsale) de la moelle spinale, séparant les moitiés gauche et droite de la face postérieure de la moelle spinale. Le sillon médian postérieur est bordé de chaque côté par des faisceaux de fibres nerveuses appelés les funicules postérieurs (ou cordons blancs dorsaux).
